Vaccine Titre Clinic Submission Form

I understand that titre testing involves a routine blood draw performed by a veterinarian and/or trained technician.

I confirm that my pet does not require special restraint or pre-sedation for this procedure or for typical veterinary visits.

I am comfortable assisting with gentle restraint of my pet for the blood draw if requested.

I understand that the procedure may be stopped at any time if the staff or I feel it is unsafe or overly stressful for my pet.

I understand that while this is a low-risk procedure, minor side effects such as bruising or mild discomfort may occur.

I understand that Dr Paris Asghari is establishing a valid veterinarian-patient-client-relationship (VPCR) for the purposes of drawing blood and interpreting results only. By participating in the titre clinic, my pet is not a patient of Dr Paris Asghari's practice and that my VPCR with Dr Paris Asghari is solely for the purposes of collecting blood and interpreting titre results.

I understand that it is a legal requirement for dogs to be vaccinated every 3 years for rabies in the province of Ontario. I understand that a titre does not act as a replacement for a vaccine, but a way to gain insight on protective antibody levels in my dog.

I will at no time hold liable the staff of Sunnybrook Holistic Mobile Veterinary Services or ADR Pet Stores for outcomes associated with my decision to obtain a blood draw and titre test

I understand that results will take up to 2-4 weeks and I will be contacted through the email I have provided on the form.
